29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Apollo Room, 115 Pritchard Hollow Road, Westfield, PA - Employees were not provided a documented step-by-step energy control procedure in the machine specific, including alternative measures for reference during servicing and maintenance activities they performed on the multi-energy sourced Milacron-Fanuc Roboshot AI Injection Molding Machines. The lack of a documented procedure allowed employees to forget the utilization of lockout on one (1) or more of the energy sources (stored electrical, electrical, water, pneumatic, and thermal) thereby exposing employees to the unexpected release of energy, on or about January 17, 2019. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
