OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) Shipping/Receiving, 115 Pritchard Hollow Road, Westfield, PA - Employees were operating the TCM Forklift Model FCB25H2 (Serial No. 74600179) without an operator restraint system (seatbelt), exposing employees to ejection and struck-by hazards, on or about January 15, 2019.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Apollo Room, 115 Pritchard Hollow Road, Westfield, PA - Employees were not provided a documented step-by-step energy control procedure in the machine specific, including alternative measures for reference during servicing and maintenance activities they performed on the multi-energy sourced Milacron-Fanuc Roboshot AI Injection Molding Machines. The lack of a documented procedure allowed employees to forget the utilization of lockout on one (1) or more of the energy sources (stored electrical, electrical, water, pneumatic, and thermal) thereby exposing employees to the unexpected release of energy, on or about January 17, 2019.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) A1 Machine, Apollo Room, 115 Pritchard Hollow Road, Westfield, PA - Each shift, employees inspected/recorded equipment readouts or cleaned the floor/equipment directly adjacent to a robotic arm that removed extruded product from the Milacron Fanuc Roboshot s2000i-330B AI Injection Molding Machine and placed the product on a conveyor exposing employees to a struck-by hazard created by the unguarded moving robotic arm, on or about January 15, 2019.   b) A2 Machine, Apollo Room, 115 Pritchard Hollow Road, Westfield, PA - Each shift, employees inspected/recorded equipment readouts or cleaned the floor/equipment directly adjacent to a robotic arm that removed extruded product from the Milacron Fanuc Roboshot s2000i-330B AI Injection Molding Machine and placed the product on a conveyor exposing employees to a struck-by hazard created by the unguarded moving robotic arm, on or about January 15, 2019.  c) A3 Machine, Apollo Room, 115 Pritchard Hollow Road, Westfield, PA - Each shift, employees inspected/recorded equipment readouts or cleaned the floor/equipment directly adjacent to a robotic arm that removed extruded product from the Milacron Fanuc Roboshot s2000i-300B AI Injection Molding Machine and placed the product on a conveyor exposing employees to a struck-by hazard created by the unguarded moving robotic arm, on or about January 15, 2019.  d) A4 Machine, Apollo Room, 115 Pritchard Hollow Road, Westfield, PA - Each shift, employees inspected/recorded equipment readouts or cleaned the floor/equipment directly adjacent to a robotic arm that removed extruded product from the Milacron Fanuc Roboshot s2000i-300B AI Injection Molding Machine and placed the product on a conveyor exposing employees to a struck-by hazard created by the unguarded moving robotic arm, on or about January 15, 2019. 29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(ii), (iii), (iv) and (v):  a) Facility, 115 Pritchard Hollow Road, Westfield, PA - Employees used spray bottles with 70% Isopropyl Alcohol and were unaware of hazards associated with the chemical due to the workplace labels that lacked the necessary signal word "Danger", applicable hazard statements "H319 - Causes Serious eye irritation" and "May cause drowsiness or dizziness", pictograms, or precautionary statements, as observed on January 15, 2019.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
