29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Even Hotel, Third floor: On or about 01/15/2019, and at times prior, the employer exposed employees to fall hazards of 22-feet, 9-inches to the lower level, in that a fall protection system was not provided to employees performing concrete formwork on the east front corner on the third floor.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:    (a) Even Hotel, Third floor: On or about 01/15/2019, and at times prior, the employer exposed employees to impalement hazards on the third level walking/working surface, in that vertical 2-ft. to 3-ft. pieces of rebar were not capped or bent over to protect employees engaged in form work and other concrete services.
