29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 25 Tucker Drive, Leominster, MA: On or about November 24, 2018 employer had not ensured that procedures were utilized for the control of potentially hazardous energy while employees were conducting covered activities such as but not limited cleaning up tailings and wood dust particles under the EB2 return conveyor while the chain and sprocket assembly skirt guard was removed. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact: 25 Tucker Drive, Leominster, MA: On or about November 24, 2018 employer had not ensured that before lockout or tagout devices were removed and energy was restored to machine or equipment it was inspected to ensure it was operationally intact to include any guards removed as a result of the maintenance activity on the (EB3) work surface return conveyor. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
