29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)   On or about January 15, 2019, the exposing employer, Machete Roofing, LLC, did not ensure employees were protected from fall hazards of up to 21 feet while engaged in residential re-roofing operations.  Due to lack of fall protection measures in place during this operation, an employee was involved in a fatal fall to the next lower working level.      Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
