29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.    The employer is failing to protect employees from egress hazards.  This was most recently documented on or about January 15, 2019, when the emergency exit route was not at least 28 inches wide by the door directly north of the employee entrance, door. The exit access was blocked by a JLG scissor lift that was stored in front of the emergency exit door.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    Employee's were exposed to struck by hazards from forklifts. Forklift operators were not trained with training consisting of formal instruction, and practical training and evaluation. There was no written and practical evaluation of the forklift operator's driving.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    Employee's were exposed to struck by hazards from forklifts. The Allis-Chalmers forklift did not have non-skid surface for the brake pedal.  The forklift was not taken out of service.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    Employees were exposed to electrical hazards.  The light switch box under the electrical panel for the oven had an electrical wire that was protruding from the side of the light switch box.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    Employees were exposed to electrical hazards.  The top junction box by the wheelabrator did not have a cover and contained live electrical wiring.
