29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  Welding Shop: On or about January 16, 2019, and at times prior thereto, welders fabricating steel containers were exposed to continuous noise at a dose of 81.8% (8-Hour Time Weighted Average sound level of 85 dBA) during the 461 minute sampling period. For a full eight-hour work shift, this exposure is equivalent to an average sound level of 88.5 dBA. Exposure calculations include a zero increment for the 19 minutes not sampled.  G & J Container Manufacturing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(c)(1), which was contained in OSHA inspection number 1327878, citation number 1, item number 1 and was affirmed as a final order on 12/17/2018, with respect to a workplace located at 14350 Lookout Rd. #5, San Antonio, Texas 78233.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  Spray Area:  On or about January 16, 2019, and at times prior thereto, employees were exposed to respiratory hazards while painting steel containers without a respiratory program to address key issues such as, but not limited to, training, fit testing, and medical evaluation for employees required to use a tight fitting half facepiece respirator.
