29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    South Elgin, Illinois plant, slitting, KYMC slitter no. 1 (model no. SMART-1300, serial no. 4281), on or about January 8, 2019, slitter operators engaged in cleaning and setup tasks on energized equipment were not provided with adequately developed and documented machine-specific hazardous energy control (lockout tagout) procedures. The procedures lacked specific steps to isolate and control hazardous energy sources, steps for the placement of lockout devices, and specific verifications steps to determine the effectiveness of the energy control measures. Operators had their hands exposed to caught-in hazards from contact with rollers' in-going nip points.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    South Elgin, Illinois plant, slitting, KYMC slitter no. 1 (model no. SMART-1300, serial no. 4281),  on or about January 8, 2019, slitter operators assigned to clean and setup equipment that required the use of lockout/tagout were not adequately trained on safe application, usage, methods, and means to isolate and control hazardous energy sources. Operators had their hands exposed to caught-in hazards from contact with rollers' in-going nip points.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:     South Elgin, Illinois plant, slitting, KYMC slitter no. 1 (model no. SMART-1300, serial no. 4281) - on or about January 8, 2019, the employer did not conduct inspections of their hazardous energy control procedures at least annually to identify and correct any deviations or procedural inadequacies. Slitter operators assigned to perform cleaning and setup activities had their hands exposed to caught-in hazards from contact with rollers' in-going nip points.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (1) prepare for shut down by recognizing energy hazards and its controls,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device,  (5) render safe all potentially hazardous stored or residual energy,  (6) verify that isolation and de-energization of the machine or equipment have been accomplished.    South Elgin, Illinois plant, slitting, KYMC slitter no. 1 (model no. SMART-1300, serial no. 4281),  on or about January 8, 2019, slitter operators engaged in roller cleaning tasks were not required to follow the established sequence for hazardous energy isolation (Lockout Tagout) of equipment, and relied on a stopped automation cycle to prevent unexpected startup. Operators had their hands exposed to caught-in hazards from contact with rollers' in-going nip points.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
