The room was provided with signage requiring protective eyewear.

On or about January 17, 2019, Torsion Department: Power transmission belt and pulley assemblies on two Sleeper + Hartley spring machines, #1 and #3, were not provided with enclosure guards. The belt and pulley assemblies were located at the base of the equipment where employees could be caught in the nip points created as the belt went around the pulley.

On or about January 17, 2019, Heat Treat: Access to the Square D 120/240 V Load Center panel behind the Branson Vapor Degreaser was blocked by the placement of a work table containing racks. The panel provided power to exhaust and pedestal fans, lights in heat treat and grinding areas, the 900-degree oven, chiller, degreaser and still.
