29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about 1/18/2019, Employees were not protected from hazardous energies on Coater #5 during servicing and maintenance activities because the coater was being operated with the fence guard door micro switch by-passed on the operators side of the machine. Line drive fence guard door did not have a micro-switch on it and the belt drive cage door for motors was being run with the bolts for the door not in place to keep the door secured. b) On or about 1/22/2019, Employees were not protected from hazardous energies on the WOR-TEX Granulator , Model: B-ISE Edge Trim Grinder during servicing and maintenance activities because there was no interlock on the hinged panel and it was missing the control arm for the micro switch.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 1/17/2019, the Pouch machine out feed belt conveyor had an ingoing nip point below the roller for HMC #4. b) On or about 1/17/2019, the Roan machine cut off roll was not guarded adequately from aisle side with a nip point on the sealing roll. c) On or about 1/17/2019, the #6 Coater wax finish ingoing nip points were not protected from access for the op rubber roll and bottom cylinder roll. d) On or about 1/18/2019, the Former slitter room machine #2 had unguarded ingoing nip points. e) On or about 1/18/2019, There were unguarded slitters on the REM Slitter, FRS #2. f) On or about 1/22/2019, there was a lack of guarding on the former section for the thermo former #7 which left access to pinch points inside the machine in three areas. g) On or about 1/22/2019, Lack of guarding for #7 thermo trim press T-130 with machine mounted approximately 29" off the floor. h) On or about 1/22/2019, the #19 Treater platform had pinch points as the machine rotated over the guard rails provided.
