29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about January 17, 2018 at the Western Forms facility located at 6200 Equitable Road in Kansas City, Missouri - Employees were exposed to shock/electrocution hazards in that a 110 V extension cord powering a portable hand grinder flexible cable had pulled loose from its attachment point leaving conductors exposed.
