29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: This violation was discovered on or about January 17, 2019, when the employer did not establish a program consisting of energy control procedures to include but not limited to employee training and periodic inspections. Employees are exposed to a caught-in and amputation hazards.

29 CFR 1910.303(b)(2): The employer did not ensure that all listed or labeled equipment was installed and used in accordance with any instructions included in the listing or labeling. This violation was discovered on or about January 17, 2019, when during the walk around it was discovered a welding machine used a makeshift power cord using a knockout box at the end of the cord to which the welder was connected. Employee was potentially exposed to electrical shock hazards.
