OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards from falling wood roof trusses.    Jobsite: The one hundred and fifty five (155) C5 Hip trusses, each weighing 441 pounds, spanning 75 feet atop a single story, 12 foot high 2 X 4 wood framed exterior walls of a commercial building were not sufficiently braced and secured in accordance with Building Component Safety Information (BCSI) requirements, Universal Component Corporation's truss manufacturer's requirements or the requirements of a registered professional engineer.  On or about January 17, 2019, while installing the 156th C5 Hip truss (last one) the entire 155 insufficiently braced and improperly secured trusses collapsed striking and pinning three workers.      Abatement Certification and documentation is required for this item.

29 CFR 1926.1053(b)(8): Ladder(s) placed in any location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, or a barricade was used to keep the activities or traffic away from the ladder.    JOB SITE: The "job made" ladder used by an employee installing roof trusses at a height of 16 feet above the ground was not secured to prevent accidental displacement, or a barricade was used to keep the activities or traffic away from the ladder.

29 CFR 1926.1060(a) The employer did not provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.   JOB SITE: Each employee installing roof trusses using a "job made" ladder at a height of 16 feet above the ground was not provided with training and information to enable the employee to recognize hazards related to ladders and stairways and in the procedures to be followed to minimize these hazards.
