Size: 335; serial #L3498) was not guarded from unattended operation. 2) The foot treadle on press brake#2 (Manufacturer: Dreis & Krump Chicago Manufacturing Co.) was not guarded from unattended operation. Violation occurred on or about 1/17/2019.
