This violation was observed on or about January 15, 2019 at 3005 S. Broadway, Edmond, Oklahoma, the employer did not ensure each employee was properly wearing a safety harness with lanyard attached to the proper anchor points while in the truck mounted aerial lift exposing employees to a fall from approximately 25 ft. to the ground below.
