29 CFR 1926.453(b)(2)(v):   A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.     This violation was observed on or about January 15, 2019 at 3005 S. Broadway, Edmond, Oklahoma, the employer did not ensure each employee was properly wearing a safety harness with lanyard attached to the proper anchor points while in the truck mounted aerial lift exposing employees to a fall from approximately 25 ft. to the ground below.
