29 CFR 1917.12: The employer did not eliminate, to the extent possible, conditions causing slippery working and walking surfaces in work areas used by employees: (a) on 2/15/2019, at the CalPortland facility located at 5034 NW Front Street, dock workers were exposed to hazards of tripping, slipping, and falling, in that the dock was slippery and the expanded metal grate used to prevent slipping was not securely attached to the dock and created a risk of snagging workers feet and causing a fall into the river.
