An employee was observed climbing onto the midrail of the basket while in the Snorkel, TB42 Articulating Boom Aerial lift basket exposing employees to a fall from approximately 30 ft. to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. 1) This violation was observed on or about January 15, 2019 at 2300 E. 2nd St, Edmond, Oklahoma, the employer did not ensure that each employee wearing a safety harness had a lanyard attached to the proper anchor points while in the Snorkel, TB42 Articulating Boom Aerial lift basket exposing employees to a fall from approximately 30 ft. to the ground below. 2) This violation occurred on or about January 15, 2019 at 2300 E. 2nd St, Edmond, Oklahoma, the employer did not ensure that each employee in an aerial lift used to access the upper floors and roof was wearing a safety harness with lanyard attached to the proper anchor points while in the Snorkel, TB42 Articulating Boom Aerial lift basket exposing employees to a fall from approximately 30 ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. This violation occurred on or about January 15, 2019 at 2300 E. 2nd St, Edmond, Oklahoma, the employer did not ensure that each employee working in aerial lifts and working at heights greater than 10 ft. were provided training in fall hazards and how to properly use fall protection exposing employees to fall hazards.
