29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): a) 550 Woodlake Cir., Ste A, Chesapeake, VA 23320: On or about March 21, 2019, and times prior thereto, the employer did not make an exposure determination for employees exposed to chromium (VI), where employees performed welding tasks on stainless steel, exposing employees to increased risk of lung cancer, airway and skin sensitizations. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.110(f)(2)(i): LP gas containers were not located so as to minimize exposure to excessive temperature rise, physical damage or tampering by unauthorized persons: a) 550 Woodlake Cir., Ste A, Chesapeake, VA 23320: On or about March 8, 2019, and times prior thereto, an approximately 20-pound propane cylinder in storage was not protected from physical damage or tampering by unauthorized persons. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
