A) Underground Maintenance Services, LTD. on or about January 18, 2019, at the address of 240 North Huron Street, Toledo, Ohio: The employer failed to ensure employees were adequately protected from cave-ins while locating utilities for a water main tie-in, in a 6 ft. deep trench. B) Underground Maintenance Services, LTD. on or about January 18, 2019, at the address of 240 North Huron Street, Toledo, Ohio: The employer failed to ensure employees were adequately protected from cave-ins while installing non-engineered temporary shoring to be utilized for a water main tie-in operation, in a 12 ft. deep trench.
