29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least three (3) treads and at least four (4) risers was not equipped with stair rail systems and handrails: At 500 Loyal Hood Industrial Park, Doniphan, Missouri, specifically inside of the Inspection Area, the flight of wooden stairs consisting of 15 risers was not equipped with a standard stair rail system. The stair rail system was missing an intermediate rail. This violation was noted on 01/30/19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At 500 Loyal Hood Industrial Park, Doniphan, Missouri, specifically inside of the Maintenance Shop, the rotating parts belonging to the ENCO Lathe, m/n LG-2080, were unguarded. This violation was noted on 01/30/19.
