29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:      Medio Springs Ranch, Building 6:  On or about January 18, 2019, an employee without fall protection was exposed to a fall hazard greater than 6 feet while working from an aerial lift without being trained on fall hazard recognition.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   Medio Springs Ranch, Building 6:  On or about January 18, 2019, an employee was exposed to a struck-by hazard while not wearing head protection as overhead roofing work was being performed.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:  Medio Springs Ranch, Building 6:  On or about January 18, 2019, an employee was exposed to a fall hazard while working on a Genie S-60 aerial lift without a personal fall arrest system.
