Building B - On or about January 14, 2018, the rungs on a wooden job-made ladder were not level due to the slanted positioning of the ladder, thus exposing employees to a fall hazard of approximately 11.4 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways. a).
