29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  2300 Palisades Ave Union City, NJ (Residential Home)    Employees were installing gutters from a work platform of a pump jack scaffold without a complete fall protection system in place, thereby were exposed to an approximate 25 ft fall hazard.     Violation occurred on or about 10/23/2018

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  2300 Palisades Ave Union City, NJ (Residential Home)    Employee accessed the roof surface with an Werner Aluminum extension ladder that extended less than 3 feet above the upper landing.     Violation occurred on or about 01/16/2019.
