29 CFR 1910.219(d)(1):  Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    At a facility located at 5007 N. Wren Drive, Appleton, WI 54914; the employer did not guard to protect operators and other employees from:    a)  The ingoing nip point of a HM TAS-150 Belt Grinder presented by the space between the abrasive material belt and the rear drive pulley of the machine.    b)  The ingoing nip point of a Kalamzaoo Vertical Belt Sander presented by the space between the abrasive material belt and the lower drive pulley of the machine.

Wren Drive, Appleton, WI 54913; flexible cords No. 14 or larger were repaired by splice; however, the splice did not retain the insulation and outer sheath properties, and usage characteristics of:    a)  A 12AWG SJTW extension cord that had its female plug replaced.    b)  The flexible cord insulation of the Ironton hoist's pendant above the Elimitint pickling tanks was pulled from the fitting.
