29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about January 18, 2019, on the 4:12 pitch roof, five employees installing a new roof system were exposed to fall hazards ranging in height from 8 feet to 13 feet from the eaves and raked edges in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.   On or about January 18, 2019, at the south edge of the carport, the aluminum extension ladder used to access the roof was extended 16 inches above the edge of the upper landing surface, exposing 5 employees to an 8 feet fall hazard.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  a.   On or about January 18, 2019, at the south edge of the carport, the roof access ladder was placed at a 63-degree shallow angle that could have allowed the ladder to slide outward, exposing employees to an 8 feet fall hazard.
