29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) At the jobsite, on or about January 18, 2018, employees used a Werner aluminum extension ladder that did not extend 3 feet above the upper landing surface to access and exit the second working level of a new commercial building during wood framing activities, resulting in fall hazards of up to 14.3 feet onto earthen ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) At the jobsite, on or about January 18, 2019, employees working on upper levels while performing wood framing activities were exposed to fall hazards of up to 21.2 feet and fall protection was not provided.    Avila Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1233082, citation number 1, item number 1 and was affirmed as a final order on June 26, 2017, with respect to a workplace located at 3165 Dallas Highway, Marietta, Georgia.
