On or about January 19, 2019, and times prior thereto, employees operating the Cincinnati Proform+ 90 - 350 ton press brake were exposed to struck-by, caught-in and/or amputation hazards from not having point of operation guarding in place nor were other methods provided to prevent exposure to the recognized hazard.
