29 CFR 1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:  (a)  Jobsite:  On or about January 22, 2019, the operator of the JLG Skytrac (Lull) was not trained to safely operate the machine.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about January 22, 2019, two employees were working from a roof and they were not protected from falling to the ground.  The fall hazard ranged from about 18 feet to about 25 feet.  The pitch of the roof was 6 in 12.  (b)  Jobsite:  On or about January 22, 2019, an employee was working from a "debris basket" that was being lifted by a JLG Skytrack (Lull) and he was exposed to a fall hazard of about 18 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a)  Jobsite:  On or about January 22, 2019, the two employees who were working from the roof and the "debris basket" need to be retained in how and when to use fall protection.
