Boiler Room:  On, or about, January 22, 2019, a wet/dry vacuum, hand cart and part of a refrigerator/dehumidifier were stored in front of the disconnect switches for the cooling towers.

29 CFR 1910.1030(c)(1)(ii)(B): The employer's Exposure Control Plan did not include the schedule and method of implementation for 29 CFR 1910.1030(d), Methods of Compliance and 29 CFR 1910.1030(f), Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up:    Facility:  On, or about, January 30, 2019, the Exposure Control Plan contained the following deficiencies:    A.  The Exposure Control Plan did not include procedures to follow to address blood spills.      B.  The Exposure Control Plan did not address post vaccination antibody testing and what procedures would be followed in the instance of a non-responder to the series.  Also, the acceptance form for the Hepatitis B vaccination series included a waiver of liability, which does not comply with the at no cost provision of the regulation.

29 CFR 1910.1030(d)(4)(ii)(A): Contaminated work surfaces were not decontaminated immediately or as soon as feasible when surfaces were overtly contaminated or after any spill of blood or other potentially infectious materials:  Facility:  On, or about, January 30, 2019, blood spills in the facility had not been cleaned using an appropriate disinfectant in that:  A.  Blood was cleaned from a carpeted area using Whittaker Crystal Dry Extra.  The facility did not supply a disinfectant that was approved for use on a carpeted surface.  B.  Blood was cleaned using Ecolab 73 Disinfecting Acid Bathroom Cleaner using a contact time of fifteen seconds in accordance with the facilities housekeeping general cleaning procedures.  The facility lacked a procedure and training to instruct staff that the disinfectant required a contact time of five minutes to be effective against blood borne pathogens in accordance with the manufacturers instructions.
