29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    a) Packing Area: Procedures were not utilized for employees who performed tasks requiring lockout including but limited to clearing jams on the Vegatronic 1000 Ilapak Bagging machine, on or about 12/22/19.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.    a) Packing Area: Machine operator employees required to clear jams on the Vegatronic 1000 Ilapak packaging machine were not affixing lockout or tagout devices to each isolating device, on or about 12/22/19.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.      a) Packing Area: Machine operator employees required to clear jams out the machine on the Vegatronic 1000 Ilapak Bagging Machine were not provided lock out tag out training, on or about 12/22/19.    Caesar's Pasta, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(7)(i), which was contained in OSHA inspection number 1270393, citation number 1, item number 1 and was affirmed as a final order on 2/28/18, with respect to a workplace located at 1001 Lower Landing Road Blackwood New Jersey 08012.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
