29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Meat Department - On or about January 30, 2019, the employer failed to ensure that established procedures were followed prior to servicing and maintenance (set up) of the vertical band saw. Employees were exposed to potential amputation hazards while assembling the saw without first disconnecting it from the power source (unplugging).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Meat Department - On or about January 21, 2019, the employer failed to ensure that the blade guard on the vertical band saw used to cut meat was adjusted to ensure the unused portion of the blade was guarded, exposing employees to amputation hazards. An employee engaged in cutting a piece of meat sustained a finger amputation after coming into contact with the blade during operation.
