A) Bruns Construction Enterprises, Inc. on or about January 16, 2019 at the address of 5404 Fort Recovery - Minster Road, Saint Henry, OH: The employer did not ensure an employee had donned and secured his personal fall arrest systems to a E400AJP Narrow JLG Aerial Lift. The employee was working from a height of 7 feet when he fell out of the basket to the ground below, resulting in his hospitalization.
