Each fire door, including its frame and hardware, must be listed or approved by a nationally recognized testing laboratory:  Front Breakroom Stairway:  On, or about, January 31, 2019, the exit stairway that provided egress from both the main level and the lower level was not enclosed at the top landing or at the landing that led to the lower level.  In the event of an emergency, the stairway could be compromised by the lack of protection from smoke and fire.
