29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided or used wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.     54 Western Ave, West Springfield, MA    On January 17, 2019 and at times prior, the employer did not require employees who operated Tabor Brasive Cutoff Machines to wear protective aprons to protect against known struck by hazards.  On January 17, 2019, an employee who was operating a Tabor Cutoff machine without an apron received chest laceration injuries when a blade disintegrated.

29 CFR 1910.215(b)(5): Surface grinders and cutting-off machines. The maximum angular exposure of the grinding wheel periphery and sides for safety guards used on cutting-off machines exceeded 150 degrees as measured at a point not less than 15 degrees below the horizontal plane of the wheel spindle. (See Figures O-12 and O-13)    54 Western Ave, West Springfield, MA    On April 1, 2018 and at times prior, employees operated two Tabor Brasive Cutoff Machines with guarding that was not capable of reducing the angular exposure of the grinding wheel periphery and sides to less than 150 degrees.  On January 17, 2019, an employee received finger amputation injuries while operating one of the two inadequately guarded saws when a blade disintegrated.
