29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. c) On January 24, 2019, employee was exposed to a caught in hazard when doing adjustments to the blanker system on the B-3 Die cutter without utilizing energy control procedures b) On or about January 24, 2019, and times prior employees were exposed to a caught in hazard by not utilizing procedures on the control of potentially hazardous energy when setting up the press 1,3,9,8 in the production area. c) On or about January 24, 2019, and times prior employees were exposed to a caught in hazard by not utilizing procedures on the control of potentially hazardous energy when setting up or un-jamming the Die Cutter B-1, B-2, B-3 in the production area.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. On January 21, 2019 Employees were exposed to electrical hazard when electrical conductors were not fully encased in the conduit and not protected from abrasions on printing press 1 in the production area.
