29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: a. 410 Bethel Avenue, Twin Oaks, PA 19014 - The employer did not ensure that an employee wearing a full-face respirator (Sperian 702007 Model Number 7620, Part Number 762000 equipped with Sperian 100400 NH3/CH3NH2 cartridges) for protection from inhalation of gaseous ammonia was fit tested annually to assure an appropriate assigned protection factor is provided. This condition was observed on or about Jan. 18, 2019.
