Location: Store, lower cabinet   On or about January 24, 2019, Employees were exposed to crushing injuries in that the employer failed to maintain and repair a locking cabinet door and discontinue use of the of a broken cabinet (case 8) that could not be secured due to a broken weld.  The employer continued to required employees to utilize the broken cabinet multiple times daily.
