29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about January 25, 2019 at the Aeroform facility- Employee was exposed to a rotating part from the aluminum lathe machine, in that the rotating part was not covered or guarded adequately enough to prevent employee from being caught-in machine.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a):   On or about January 25, 2019 at the Aeroform facility- Employee was exposed to serious lacerations and or struck-by injuries, in that the Milwaukee Portable Hand Grinder did not have a 180 degree guard that protected hands from the cutting wheel.
