29 CFR 1928.57(a)(6)(iii): The employer did not provide instruction to employees to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. On or about January 18, 2019, and at times prior there to, the employer did not ensure that employees performing cleaning duties on a lint cleaning machine were instructed to disconnect power and wait for the machine movement to stop before servicing the equipment. Employees were exposed to caught-in-between and amputation hazards.

29 CFR 1928.57(a)(11): The employer did not ensure that whenever moving machinery parts present a hazard during servicing or maintenance, the engine was stopped, the power source disconnected, and all machine movement stopped before servicing or maintenance was performed. On or about January 18, 2019, and at times prior there to, the employer did not ensure that employees performing cleaning duties on a lint cleaning machine, had disconnected power and waited for the machine movement to stop before servicing the equipment. Employees were exposed to caught-in-between and amputation hazards.
