29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about January 21, 2019, employees were exposed to crushing injuries in that the Line 2 #1 Fleetwood 56-D Rotofeeder was not locked out prior to performing the removal of steel can ends, resulting in a crush injury to the forearm. The employer did not ensure that energy control applications steps to control the electrical energy sources were implemented prior to maintenance tasks; the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished.

On or about January 25, 2019, the employer did not ensure employees were protected while operating the #1 FSI Fleetwood Rotofeeder (Model 56D). Employees were required to operate the rotofeeder and are exposed to pinch points at the discharge end of the feeder as it indexes.
