On or about January 3, 2019, on the 2nd level balcony, employees were not protected from a 13 feet exterior fall hazard by the use of a fall protection system. b. On or about January 3, 2019, on the 2nd level hallway, a 5 feet by 5 feet opening was not adequately guarded, exposing an employee to a 13 feet interior fall hazard.
