29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about November 28, 2018, the employer failed to report within 24 hours a work related injury resulting in the partial amputation of the employee's left thumb. The incident occurred on November 28, 2018 and was reported to the Baton Rouge Area OSHA office on December 6, 2018.
