29 CFR 1910.179(e)(2)(i): Crane(s) were not provided with bridge bumpers or other automatic means providing equivalent effect: a. Plant 2 Bay 6 - On or about January 23, 2019, the employer did not ensure the bridge bumpers were provided on the Abell-Howe 10 ton top running overhead crane (serial no. MC66597-1). Employees were thereby exposed to hazards associated with unstable loads in the event of cranes colliding.

29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections: a. Plant 2 Bay 6 - On or about January 23, 2019, the overhead cranes operated by employees were not inspected at frequent (monthly) intervals. Items not inspected included, but not limited to, functional operational mechanisms, hooks, hoist chains, and running rope. Employees were thereby exposed to struck-by hazards. The overhead cranes as follows: Plant 2 Bay 6 - Illinois Crane 5 ton (serial no. 1701431) top running overhead crane. Plant 2 Bay 6 - Abell Howe 10 ton (serial no. M66597-1) top running overhead crane.
