29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l)  (Construction Reference 1926.602):  a) Throughout Jobsite - On or about January 24, 2019, and at times prior to, the employer did not ensure that all employees who operated powered industrial trucks (PITs) were competent to operate PITs safely and had completed the training required by this paragraph (l), thus exposing employees to struck-by and fall hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Roof - On or about January 24, 2019, the employer did not ensure that employees were protected by safety net systems, personal fall arrest systems, or by guardrail systems while installing tiles onto the building's roof, thus exposing employees to a fall hazard up to about 15 feet.
