29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give the maximum protection possible for the operation being performed.  (a) Packaging - On or about 11/14/18, employees were using a DeWalt radial arm saw on which the lower portion of the blade was not guarded, exposing employees to amputation hazards.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  (a) Throughout facility - On or about 11/14/18, the employees expected to use portable fire extinguishers were not provided with training at least annually, exposing employees to fire hazards.  Aluminum Metalworks, Inc (predecessor of Champagne Metals) was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1910.157(g)(1), which was contained in OSHA inspection number 1119110, Citation number 2, Item number 1 and was affirmed as a final order on 3/10/16, with respect to a workplace located at 3100 MacCorkle Ave SW, South Charleston, WV.
