29 CFR 1910.145(c)(2)(ii): Employees were not instructed that caution signs indicate a possible hazard against which proper precautions should be taken: On January 10, 2019, an employee sustained a head injury requiring hospitalization as a result of overreaching on a Cotterman Easy 50 Mobile Ladder for car parts. The overreaching caused unstable footing resulting in the employee falling backwards down the ladder. The employer did not trained the employees on the caution sign posted on the ladder that stated "overreaching could result in a fall, descend and relocate closer to work."

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On January 10, 2019, the employer did not report the in-patient hospitalization of an employee who sustained a head injury from falling off a mobile ladder within 24 hours. The employee was hospitalized for 15 days for the incident and has not returned to work yet. OSHA was not notified until January 24, 2019.
