A written respiratory protection program containing the requirements standard elements such as but not limited to worksite-specific procedures, respirator selection, employee training, fit testing, medical evaluation, and respirator use, cleaning, maintenance, and repair had not been developed or implemented by the employer.

The incident occurred on or about 1/17/2019 at 9:00am.  The employer did not report the incident to OSHA prior to the initiation of the inspection on 1/25/2019.
