29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    a) 300 South Ave Ste 9 Garwood, NJ, 07027 - Grocery and Dairy Departments    The employer did not provide employees using forklifts in the workplace with a forklift operator evaluation every three years.    Condition observed on or about 01/25/2019.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they are readily accessible during each work shift to employees when they were in their work area(s):  a) 300 South Ave Ste 9 Garwood, NJ, 07027  The employer did not provide employees with updated copies of required safety data sheets which were consistent with the UN Globally Harmonized System of Classification and Labelling of Chemicals (GHS) and included at least the section numbers and headings with associated information under each heading such as but not limited to signal words, hazard statement(s), symbol(s) and precautionary statement(s). Employees used hazardous chemicals including but not limited to quaternary sanitizer containing ethyl alcohol and alkaline detergent containing tripotassium phosphate to perform work duties.  Condition observed on or about 01/25/2019.
