29 CFR 1910.147(c)(4)(ii)(B): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a) Planer Area, Saw Main: On or about January 28, 2019, and at times prior, the employer exposed employees to electrical and struck-by hazards in that no specific procedures to control hazardous electrical energy were developed for servicing a Saw (Corley Serial Number NMT-020).

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) Warehouse: On January 28, 2019, and at times prior, the employer exposed employees to struck-by hazards in that Forklift (Caterpillar Model 494R Serial Number 10) had a frayed hydraulic hose located at the mast but was not taken out of service.
