On or about January 29, 2019, one employee was exposed to a fall greater than 4 feet when employee slide steel down from the steel shed mezzanine.

29 CFR 1910.133(a)(5): The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed, for protection from injurious light radiation. On or about March 04, 2019 employee used an oxy/fuel cutting torch to cut metal and was not wearing a eye protection with a filtered lens that would protect employees eyes from injurious light radiation.
