29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points: Location: 531 Bryant Ave Bronx NY On or about January 24, 2109, employees were working in the back and left side rooms were exits were obstructed with sheet pan racks. There were multiple racks and they were constantly moving and left blocking exit access for employees towards the back of the rooms. Exit access was completely obstructed when the hot oven doors on the left hand side of the building were open with sheet pan racks in the walkway.

29 CFR 1910.263(e)(1)(viii): Every mixer was not equipped with a full enclosure over the bowl which is closed at all times while the agitator is in motion. Only minor openings in this enclosure, such as ingredient doors, flour inlets, etc., each representing less than 1 1/2 square feet in area, shall be capable of being opened while the mixer is in operation: Location: 531 Bryant Ave Bronx NY 10474 a). On or about January 24, 2019, three horizontal mixers were unguarded while two employees added ingredients over the bowl while the agitator was in motion. One employee stood over the mixture watching bowl while other employee added the ingredients. Both employees stood directly over the unguarded mixers while the agitator was in motion.
